    Good Morning my dear friends,

    Ahhhhh beeeeeudiful rain falling here and has done for the past coupla days. I am so thankful we are getting this now as our annual rain fall is well down under 14".. All good things come to those that wait!

    Had another super busy day yesterday. Have to drive to a larger hospital today to visit one man who is dying but was unable to stay at home due to the care he required.

    I kinda got miffed yesterday as this gentleman is end stage CHF. The doctor had ordered IVI Lasix 160mgs daily PLUS his oral lasix in the PM.

    This guy has Anasarca GROSS and I mean Gross. Skin splitting oozing, chest pain Hypotensive to the tune of 68 Systolic, HR 140, RR 40.

    So I puffed my chest out ruffled my feathers and phoned his nurse. Told her to tell him (politely) he needed to take ownership here and stop this crazy treatment. Let this man die a dignified and as comfortable as possible death.

    She phones me back saying Dr #### wants to speak with you. Ok I will admit the heart rate increased JUST a taddy but then I thought stuff it. I have to speak for this man.

    Soooooo I picks the phone up and speak with the nicest most caring Doctor. I state my case and tell him we have to look at the benefit versus burden. This man is dying he knows he is. Please I tell him lets just do this.

    Told him I had commenced Morphine with Ativan. I found out late last night he has stopped all medication apart from the Morphine and Ativan. Spoken the pts wife to tell her he is dying.

    The son does not get it and went off a wee bit when the treatment had stopped. He requires so much emotional support and understandably so this is his daddy.

    I pray for his death to be dignified. I know the Lord will take a very faithful servant home.

    He certainly has done his time here on this earth and suffered so.

    Ok I needed to get that out I suppose. I sit and cry because we wanted to keep him home but sometimes you do what you need to do for the betterment of the patient and family.

    Have a great day and please watch the roads. It IS a madhouse out there.

    Those people that are near or involved in the fires in CA please be safe and I pray for this to be over soon.

    (((Sabby)))) sometimes the best thing we can do for our pts is advocate for what is best for THEM.. not the family or the doc.
    I'm sure he thanks you ....

    not much going on today... tired from watching most of the game.

    work (short again) then respite. then game 2.

    will need to sleep more this weekend to catch up!

    *Groan* Good morning---had set my alarm for 6am, woke up at 3am feeling rather unhappy and stressed (bad dream), then woke again at 5am so I got up. I have an interview for a seasonal position at a nearby Carson Pirie Scott in a few hours and I'm trying to wake up. Got to meet with a local community organization tomorrow morning as part of NS. I have/had something going on every morning this week, and it's tiring--thank God I only have a Pharm quiz to study for next week.

    Anyway, I've complained enough. Let's just say I'll be really glad when Thanksgiving hits and all have to do is spend a few hours selling men's dress socks or something.

    Sabby, I understand why it'd be tough to confront the MD, but you did the right thing. And I'm glad that I could glance at the VS and think, "Does this guy have CHF?"---I've actually learned something!

    I'm distressed and angry. We lost yet another soldier in Afghanistan!
    My heart breaks for that man and his loved ones. It's TOO CLOSE to home! As members of the wider Australian military family, you feel this deeply.

    Please say a prayer for all those serving.

    Sorry to dump this here. I just feel SO sad and angry tonight.
    I wonder when will it all end? The sensless idiotic killing and hatred.
    Why can't the human race learn lessons from the past?

    It's OK Grace, we all feel that way at some point. Hard as it can be to realize sometimes, you will feel better and the war will end someday.

    yup let's pray for people in need! yesterday i was much than impressed when the teacher of my dd asked me were is daddy of my dd, because at the prayer hour when she was asked for who she wants to pray my dd said :"for my daddy to come home from the christmas!" ans he is not in war. taught yeah??? priorities - our children! so be good girls and boys in souls and minds!
    so how zuzi believes in magic she used a trick to find "the way" and run with her car to see if her green stop lights are opened like always in her californian life....sooooo..it seems that yes ....still there my green californian lights. loooooool, zuzi up, need to go, hugs and prayers for all in need and a happy song (my loving cher and sonny

    Now esplain to me, you wanted to stop the Lasix why? I would have thought it would decrease the anasarca. Do you mean the discomfort of frequent urination or ... ??? Sorry, it is early for me, lol...

    "You could say that I am a health nut," Benny answered. "I have never smoked or drunk alcohol, I am always in bed by ten o'clock, I've been going to Israeli folk dance classes since I was a teenager and I've always walked three miles a day, even in rain or snow."

    "But," said the reporter, "my uncle Saul followed exactly the same routine and he died when he was 75. So how come it didn't work for him?"

    "All I can say," replied Benny "is that he didn't keep it up long enough."

    Grace--prayers to the lost soldier and his family. So disturbing to me to put on the news and hear about more soldiers killed in Iraq/Afghanistan.

    My exciting day today--a couple of dental extractions as part of a plan for a partial upper. My teeth have always been bad and I knew I was heading in this direction but the time has come. No more trying to save what can't be saved. Oh well. Nothing shocking to me...I'll adapt.

    Wonderful baseball game last night. I hope Boston continues to hit as they did last night. :spin:
