How Horrible, Space Shuttle Columbia is breaking up and it appears that it may crash in the Dallas area, debris is scattering and is considered toxic, OMG! I am watching this on tv with tears in my eyes, I just can't believe how tragic this is.

It was going at 12,000 miles per hour when contact was lost. If even one of those heat resistant tiles is loose or worn and it overheats you got trouble. Also if you hit the outer atmosphere at those speeds at wrong angle you got trouble. I doubt the later, those pilots are just to good at what they do to make that kind of mistake. Six or seven crew members on board, most likely all lost. Most important that people in Florida do not touch anything that falls to ground. Fuel will cover everything and is extremely toxic. There is areospace engineer on tv saying possibility of surviors cannot be ruled out. From the pictures I would say that is a remote possiblity.
