Back in the Saddle!!!

  1. Woo, baby!!! Started my new job last week at a local hospital. I am LOVING IT!!! I had not realized how much I have missed being in the acute care environment until I hit the floors, smelled my first GI Bleeder, heard a code being called, listened to a screamer... ahhhhhhh.... YES!!!!!!!!!!

    I'm working in the capacity of what used to be called a discharge planner, except it's more multi-faceted now. We have social workers we work with who also function as case managers (they are WONDERFUL!!!) and the nurses are assigned to units. So far it looks like I'll have the SICU and Cardiac post-procedures.

    So, what do I do? Try to expedite the transfer and/or discharge of patients who no longer meet criteria to stay either in ICU or in the hospital. Decrease bed days. This involves coordinating with outside agencies and facilities (i.e. LTAC'S, home health agencies, drug/alcohol rehab, inpatient psych, etc.). This involves patient with insurance, patients with NO insurance, Medicare, Medicaid, etc, and having to know & understand what is covered and not covered. It also involves having to work very nicely with physicians: Telling them when their patient no longer meets criteria for being in the hospital, requesting orders for PT/OT eval, and asking for orders to transfer or discharge, just to name a few.

    What I see a lot is that there are often so many consultants on a case that they are not seeing what each other is doing and SOMEONE has to be the liason between all of them to get the desired outcome of the patient's stay. This has become so much easier now that we have physician's direct cell phone numbers and they are much more open to assistance from the hospital case manager than before.

    I love this - I love interviewing the patients, I love working with the physicians and ALL the hospital staff - nurses, physical therapists, speech therapists, etc. My first two days were happily exhausting - I went from sitting behind a desk in an office for 11+ years to walking all over the hospital and using the stairs!! Sore! But that's good! Frankly, I need to walk to Maine and back with this large azz of mine, but this is a good start!!

    Carrying two cell phones and a laptop is way cool. Technology has made this job MUCH easier than years before. I don't have internet access at work so I won't be on as often (some of you are saying "Yay!" - cut that out :chuckle) but I'll be checking in when I can.     I had wondered where you've been. Glad to hear your job is cool. A friend of mine works in that same capacity and also loves her job.

    Sounds really interesting, too. I do have a question though. Does your dad think you're a nurse again now with this job?
